SAMBA Survey Distribution Policy

SAMBA will occasionally receive requests to disseminate surveys to the membership. Distribution of surveys will happen no more than once per quarter – evaluated on a first come, first served basis. The survey requests are reviewed by the SAMBA Executive Committee with the authority to delegate to a SAMBA Committee if the survey relates.

Survey Distribution Fee: $250.00
The fee must be paid in full before the survey is disseminated.

At least one member of the investigating team must be a member of SAMBA.

The following information is required to be considered for distribution:

  • Listing of the survey investigators, including their contact information and affiliations.
  • Up to 150-word summary describing how the results of the survey will advance the art and science of ambulatory anesthesia.
  • A copy of all questions being asked in the survey.
  • Identification of the IRB that approved the survey.
  • Indication of what segment of SAMBA membership should receive the survey (e.g., active members, trainees/residents, international members, non-physicians, retired).
  • Contact name(s) for recipients to address any questions or concerns.
  • An estimate of the time it will take to complete the survey.
  • The deadline to complete the survey.
  • Desired/preferred date(s) of distribution and follow-up reminder, which are subject to final review by SAMBA staff.
    • There will be a disclosure with distribution of the survey it is not endorsed by SAMBA.

At the conclusion of the survey, complete survey results or an abstract must be available for posting on the SAMBA website to members.

SAMBA will distribute selected surveys to members by email. No names or contact information of SAMBA members will be released to investigators.

All materials should be sent to the SAMBA Executive Team at [email protected].
